Getting Ready for Your Appointment
1. Call to Confirm
Our office will contact you prior to your appointment to confirm the time and day. Please allow a minimum of 24 hours’ notice if you will be unable to keep your appointment.
2. Don’t Forget ID & Insurance Cards
We are required to obtain copies of an acceptable form of ID and your insurance card.
3. Download, Print & Sign This Form
To make your life a bit less hectic, we’ve made available online the HIPAA you need to complete.
